What is the Care Card?

	The Care Card, more than an ID document, is a personalised card offering a range of exclusive benefits to its users. This card is personal and non-transferable allowing the immediate identification of patients in the Hospital information system. During the admission at the HPA for any medical service you will have to present another document of identification.

What should I do in case of theft or loss? You should contact the Hospital Particular do Algarve immediately, stating that the card has been lost or stolen. You can either phone 282 420 400 or report it personally at the Hospital main reception. The card will be cancelled and a new card will be issued and posted to the Care Card User. Which are the benefits for my family members? Spouses and descendents will not be able to use the Care Card because it is personal and non-transferable. In order to be entitled to the Card benefits, relatives should fill in an application form, following the instructions mentioned above, and will get a special registration value price of: 35€ for the 1st relative registration 25€ from the 2nd relative registration (inclusive)
